Personalised care is a holistic and collaborative approach to meet the health and social needs of an individual or family. The care is tailored to the needs and circumstances of their situation.

Our Team

The Personalised Care Team is made up a wide range of professionals in the following fields:

  • Care Coordination 
  • Social Prescribing
  • Occupational Therapists
  • Health and Wellbeing 
  • Safeguarding

The Personalised Care Team supports all seven surgeries within the SHaR PCN.

How can we help you?

Contact your GP surgery reception to be referred to the Personalised Care Team. We will then reach out to you to have a discussion about how we can best support you.
